Service Description

Service must be paid in full upon booking online. The service cost of $30.00 will only be refunded if appointment is cancelled within 24 hours. There will be no lost of deposit if appointment is re-scheduled within 2 hours of initial scheduled time. All deposits will be refunded in 5 business days. Patient medical intake form must be completed and submitted to office prior to consultation. LATISSE® is an FDA-approved treatment to grow eyelashes for people with inadequate or not enough lashes and can only be obtained with a prescription. Consultations for treatment is offered through our telemedicine service. Consultation fee does not include cost of medication and must be paid at the time of booking your appointment. Loss of consultation fee if appointment is not cancelled within 24 hours. Prescription is sent directly to your pharmacy (assistance with prescription coverage offered).
