Service Description

A $10.00 deposit must be given at time of booking. Deposit will be deducted from total cost of initial visit at time of visit. The $10.00 deposit will only be refunded if appointment is cancelled within 24 hours. There will be no lost of deposit if appointment is re-scheduled within 2 hours of initial scheduled time. All deposits will be refunded in 5 business days. Your first visit with a medical provider will last about 30 minutes and includes the following: ​Completion of your health history and physical assessment form Scheduling of Virtual Consultation with a Health Coach Scheduling of Virtual Consultation with a Registered Nutritionist Vital sign measurements Full-body composition analysis that details your body weight, body fat percentage, body water percentage, muscle mass, body mass index, and recommended daily calorie intake Prescription medication evaluation to determine the necessity of weight loss drugs in your situation
